​​​This is an internet-based application designed to receive applications for certified copies of death records within the State of Nebraska.

The Department of Health & Human Services, Division of Public Health Vital Records is responsible for providing certified copies.

When entering your password, please be sure your CAPS LOCK is turned off.
Funeral homes will now only be able to request death certificates for one year after the filing date of the certificate. After one year the families will need to request certificates directly from Vital Records.

Funeral homes can only request death certificates for death records that they did the service for or was the trade call.

If this death is pending investigation an abstract of death will be issued. This transaction will not be refunded or exchanged.

New User's will need to register with the State of Nebraska Enterprise Registration System. The user will create a username, password and security questions to reset the password if needed. Please use "First Time User" button to register with the State of Nebraska. User will be directed to the Nebraska Enterprise Self Registration site.
